Over 75’s Specialist Nurse Practitioner Service

In 2015 Green Dreams was asked by the CCG (Clinical Commissioning Groups) to put a Specialist Nurse Practitioner service into place, to serve all the residents of care homes and nursing homes in Burnley and Padiham. The service was commissioned by the CCG and was provided on behalf of all GP Practices in Burnley and Padiham. This was in addition to the care provided by the homes themselves, the district nursing services, the GP practices and other statutory agencies. This was all done through The Green Dreams Project CIC.

 

6 full time Advanced Nurse Practitioners were employed along with administrative support and clinical supervision, to undertake regular ward rounds as well as see patients acutely. CQC (Care Quality Commission) registration was put into place for Green Dreams. The results of this service were transformational for local health services and served as a model of supportive care. When up and running the service was passed to another local provider in 2018 to continue the everyday management. It is still in place to this day.
